Two witnesses are being sought after a police-involved incident that left one man dead in Terrace Thursday night.

Aidan Buckley with the Independent Investigations Office of B.C. says his office has learned a woman may have captured video of the incident and believes a man may have watched it unfold from a distance with binoculars.

Around 1:30 p.m., Aug 18, police say they received a call from a woman at Mills Memorial hospital reporting a man walking toward her with an axe, who she said might try to hurt himself.

Officers arrived, but police say during during attempts to negotiate with the man, a conducted energy weapon, also known as a Taser, was deployed. Police say the man received immediate medical attention but later died in hospital.

The IIO says a team of nine investigators is looking into the incident.

It's asking the two potential witnesses and anyone else with information to contact them at 1-855-446-8477.
